Nerve sparing RPLND: technique

Involvement of retroperitoneal lymph nodes is usually first and common metastasis way for testis tumours. Retroperitoneal lymph node dissection is a safely applied treatment choice for clinic Stage 1-2A non seminomatous germ cell tumour and residual mass after chemotherapy.When applied bilateral and no nerve sparing,its most signifcant morbidity is loss of antegrad ejaculation and potential infertility. Acceptable antegrad ejaculation rate was obtained with introduction of modified templates. But significant improvement of antegrad ejaculation was obtained with application of nevre sparing RPLND. Nerve sparing RPLND is a surgery obtained very low recurrence and high antegrad ejacuation rate in recognition retroperitoneal anatomy and its variations, with efficient exposure and nevre sparing.
